Resume: The extent of thyroid surgery (total vs. subtotal thyroidectomy) affects the subsequent functional state of the gland and the need for replacement therapy. Objective: To compare the frequency and nature of thyroid dysfunction in the early and late periods in patients who underwent total and subtotal thyroidectomy. A retrospective analysis of clinical, laboratory, and instrumental data of patients operated on in the period 2015–2024 (n = 184) was conducted. The levels of TSH, free T4 (FT4), T3, and TPO antibodies, clinical manifestations of hypo- and hyperthyroidism, as well as the prescription of replacement therapy were assessed. Comparison of groups by the extent of surgery (total - TT, subtotal - CTT) was performed statistically (χ², t-test, logistic regression). The risk of developing clinical hypothyroidism is statistically higher in the subgroup with STT and concomitant autoimmune thyroiditis and a smaller volume of residual tissue.
